Commissioner of Oaths

We offer Commissioner of Oaths services. Certain documents require a Commissioner to certify that a required oath, affirmation or declaration have been administered properly. Our Commissioner of Oaths will witness the signatures of people who are affirming/swearing the contents of a document to be true. If you are seeking these services you must visit the Clerk's Division in person during regular business hours. Please provide valid photo identification to have your signature witnessed by a Commissioner of Oaths.

Fee

The fee is $29.38 and is payable by debit, credit card or cheque. Note: There is no charge for the Commissioner of Oaths service for Change of Name forms.

• The initial fee of $26 + HST covers up to three documents for commissioning in a single visit.
• $5 for each subsequent document for commissioning in a single visit.

Documents the Municipality will Commission (including, but not limited to):

• Domestic and Foreign Pension documents also known as Proof of Life (performed free of charge)
• Canadian Citizenship forms
• Delayed Statement of Live Birth
• Name Change (performed free of charge)
• Vehicle transfer of ownership to a family member
• Child Travel Consent
• Invitation letters for a visitor's VISA
• Immunization exemption document
• Statutory Declaration of Lost Identification
• Statutory Declaration for OSAP – Statement of Common Law Status/Supporting Children
• Amendment to Birth/ Death/ Marriage Certificate forms
• Affidavit of Proof of Loss of Motor Vehicle/boat (from an insurance company)
• Affidavit of Proof of Residency
• Travel Industry Council of Ontario - Affidavit of Customer Claimant

Documents the Municipality cannot Commission:

• Wills, Living Wills, Codicils to Wills or Estate Settlement documents
• Power of Attorney
• Divorce, separation, marriage or cohabitation documents
• Custody documents
• Documents related to debt
• Real Estate documents
• Court, Legal or Civil issue related documents
• Declaration or Claimant and Indemnity
• Statutory Declaration of Progress Payment Distribution – Holdback Security Deposits
• Statement of Arrears
• Certified True Copies
• Declaration of a Translator
• Age of Majority or BYID Card Application
• Documents requiring the signature of a Canadian Notary Public

We reserve the right to refuse commissioning services of any document.

Certification of documents

If you require certified true copies of a document, you will need to contact a private lawyer or a Notary Public, as the Municipality does not provide these services.
