To qualify for the Fee Assistance Program, you must submit copies of the following current documents with the application form:
Proof of income
You must provide a copy of one of the following documents:
- Goods and Services/Harmonized Sales Tax Credit Notice
- Canada Child Benefit (CCB) or Ontario Child Benefit (OCB)
- Ontario Works monthly statement (including a statement of deposit with address)
- Ontario Disability Support Program monthly statement (including a statement of deposit with address)
- Canada-Ukraine Authorization for Emergency Travel
Proof of residency
Include a copy of one of the following documents with your application:
- Valid Ontario Driver's Licence
- Utility bill (hydro, gas, water)
- Valid Ontario Photo Card
- Property tax bill
- Insurance policy (home, tenant, auto)
- Mortgage, rental/lease agreement
All supporting documents must be from the current tax year.
Ontario Works and Ontario Disability Support Program must be from the current month.
